Entries to be submitted either on-line, or email and can be made directly by the doctor or on his behalf by an official distributor.
Slit Lamps should be older than 20 years and in regular use. The age of the Slit Lamp can be determined by the serial number on the cross slide.
The entry form should be completed correctly and should state if the instrument is owned privately or by an institution.
The closing date for entries will be October 10, 2008 and the results will be announced at the AAO 2008 in Atlanta. Details of wining entries will also be posted on haag-streit.com website.
One entry can be made per slit lamp.
Sending e-mail is not proof that we have received your entry. No responsibility can be accepted for entries that are lost or delayed, or which are not received for any reason.
By entering the competition the winner agrees to participate in such promotional activity and material as Haag-Streit AG may require. All Images and comments can also be Used for HS promotional activity.
No part of a prize is exchangeable for cash or any other prize
